What does the damned skull do at the top?

The damned skull is a wild object. It can revive your teammates, but there is a big catch. As soon as you use it, your friends will be revived, but you will die instead. It's pretty funny, but frankly, TOP are full of stupid items like this.

When your teammates come back, they will have full endurance and all their negative effects will disappear. So you basically make a big victim to save the entire team. But don't worry, because your teammates can still get you back after you sacrifice yourself.
