Izuku Midoriya's carnage form is the result of a last-minute attempt to break through Tomura Shigaraki's iron-clad hatred and determination. In this unique transformation, Midoriya endures crippling pain and overwhelming strain, every movement a battle against his own body as he struggles to breathe and maintain control. Despite the odds, Midoriya's carnage form is the best way to execute a wild plan against someone like Shigaraki who possesses decay and rapid regeneration.
What makes Izuku Midoriya's carnage form stand out to fans is its unsettling, almost villainous appearance inspired by the Marvel villain of the same name, Carnage. Even Shigaraki himself thinks Midoriya looks villainous in this form, almost like he's not the hero trying to protect the world with everything he's got. Despite his terrifying appearance, Midoriya's carnage form is yet another manifestation of his unwavering determination to not only punish evil, but also save Shigaraki.
Additionally, Midoriya's carnage form is also known as Overlay, a name Horikoshi gave to presumably emphasize the textured chaos of black whips and black lines appearing throughout his battered and broken body. While his transformation looks crazy at first glance, beneath this exterior lies pain and suffering that Midoriya silently endures in order to survive until the end.

Blackwhip keeps Midoriya Izuku alive and moving
Without the Blackwhip, Izuku Midoriya's carnage form would never be able to fully manifest. In this transformation, the Blackwhip acts as both a lifeline and a weapon, allowing Midoriya to continue to move and breathe even when his body is pushed to the edge. Banjo's quirk represents Midoriya's limbs, muscles and tendons, compensating for the extreme strain and damage of Gearshift's recoil and Blackwhip tendrils.
My Hero Academia fans even compare Midoriya's carnage form to a super powered wheelchair, just to explain the form in simple terms. This is a grim yet fascinating image that accurately describes how the Blackwhip supports and prolongs Midoriya's weakened body in carnage form. In addition to the pain and agony, Midoriya's carnage form prevents him from being immobilized, allowing him to continue fighting.

Besides Blackwhip, the other OFA user, Kudo, was the one who strongly encouraged Midoriya to keep fighting and moving in carnage form. He knew that fighting Shigaraki, who just absorbed the AFO head-on, was a losing battle at this point, so it's best to come up with a different yet effective plot to break through Tomura Shigaraki's solid mass of hatred.

Despite the menacing appearance of Izuku Midoriya's carnage form, fans know that it is not the strongest of his transformations. After all, Midoriya's carnage form is mostly powered by Blackwhipwhich is why many other fans consider other forms, such as Full Cowling at 100% or other One For All enhancements, to be more destructive in direct combat. It's just that, in the final battle against Shigaraki, Midoriya's carnage form was the perfect choice.
While in carnage form, Midoriya successfully transferred the remaining OFA properties to Shigaraki through blood transfer. This allowed the remnants to influence internally, simultaneously forcing Shigaraki and Midoriya's memories to merge, instead of sticking to the original plan of defeating Shigaraki with Midoriya's sheer strength. If Midoriya hadn't taken a risk at this moment, he would likely have been defeated as Kudo expected.
