The romance Denji experiences in life is more of a survival horror, rather than a romance. The reality of Chainsaw man world is that when a girl takes an interest in him, there's a 99 percent chance she's really trying to take his heart out, in his case, literally. His life is marked by a desperate, starving desire to be loved, a byproduct of being a kid growing up in a shack with only a chainsaw dog and a mountain of debt. While she is certainly the most important woman in Denji's life, along with Makima, Power takes last place in the list of romantic interests for one simple reason: they are brother and sister in every way. After the International Assassin's bow trial, the two were allowed to have an intimacy, but Denji was far beyond that, and because he could emotionally connect with Power without it being romantic.
They would share the bath and even sleep together, not as a result of lust, but because they were two damaged children who feared the dark. Denji himself openly understands that he loves Power, but without any romantic feelings at all, and that their spark is not physical. It is the only relationship that Denji was selfless about; a platonic love that has defined his new vision of what it means to love someone without reciprocation.

Makima
Makima is the epitome of toxic manipulation. For her, every person is just a means to an end. She was never in love with Denji, but with the chainsaw man inside him. Denji was just a leash for her to hold on to her idol. She tapped into his touch-starved instinct, assured him of a normal life, and favors only to methodically destroy everything he's ever loved. Her romance was a masterpiece of grooming and mental torture, intended to destroy his spirit so she could possess the Hero of Hell. She is the weakest bond Denji ever had, as it was based on a lie – a parasitic bondage where he was given the impression that he was a dog as opposed to a human.
Himeno
Himeno is a complex character in Denji's life. Being an experienced hunter who had lost too many partners, she was in need of a relationship rather than the need to attend another funeral. Her “interest” in Denji was quite unethical, given the age difference and her actual love interest, Aki, was too blinded by revenge to ever notice her.
But despite the red flags, she was among the very few people who treated Denji like a human being. She did not desire his heart as a quest, but as a companion to help her forget the “easy revenge” that had consumed her life. Had they been the same age, she might have been the only woman who had ever shown him a stable, civil kind of love, even if it might have been accompanied by an excess of cigarettes and mutual trauma, but hey, that's part of the charm of being in love.

Kobeni Higashiyama
The fact that Kobeni is included as a love interest is almost accidental, on behalf of the Pochita version of Chainsaw Man. The date they went out was a nightmare scenario that was unreal as she was forced to participate in a model of a regular outing in a state of fear for her life.

To be fair, Kobeni is a literal reflection of Denji, but in an abstract way. No one actually understands the plight of being a victim of the devilish world, as she is the only one who knows what it's like to be a chess piece. While the romantic chemistry was never present, their time together at the burger joint was a rare case of two individuals being in their mutual misery and not having to go through any grand schemes that Denji is used to when interacting with women.
Fumiko Mifune
Fumiko is another kind of terror in Denji's life: the obsessed admirer. She is a devoted supporter of the Special Branch of Public Safety and claims to be a huge fan of the Chainsaw Man to the point of defending Denji in her own twisted way. Her interest isn't stronger than Makima's except for the fact that she seems to like the chaos Denji causes, but that's superficial. She is drawn to the “hero” and “legend” and often ignores the real teenage boy struggling to support a pack of dogs and a little sister. She serves as a reminder that even though Denji is liked, it's usually not for the right reasons.

Asa Mitaka
It's with Asa Mitaka that Denji has finally encountered something that could be considered more of a true high school romance, however messed up it is. They are both outcasts in society and they are not aware of how to act in a normal society. Their date at the aquarium was a nightmare of social anxiety and awkwardness, but it was real and their connection felt genuine.
Asa didn't come to Denji like Makima or Reze, as a trained agent or a manipulator at first, but just as a lonely girl looking for someone to relate to. Their relationship is based on the mutual energy of being alone, which is why it's one of the most relatable and powerful romantic possibilities in the series, as it's based on two people struggling with how to be human together.
Yoru
Yoru's interest in Denji is inseparable from the interest in Asa, but it has its own weight. Being the War Devil, her job is to kill him, but when she is connected to a brain of Asa, she experiences the same heartbeat fluttering and real attraction that Asa does. This makes it an interesting and high-stakes game where the interest is literally the struggle between the desire to destroy and the desire to love. Yoru's emotions are strong because they are beyond her control; she is a Horseman of the Apocalypse and is corrupted by the most human emotions of the girl she inhabits, and their relationship is the strongest and most dangerous in the manga.
Reze
Reze is the most influential love interest in Denji's life since she was the first to give him an option. Although her first intention was a mission in the Soviet Union, their time together, the night swimming, the cafe lessons, and the laughs they shared was the first time Denji experienced a spark that wasn't about his heart in the chainsaw. Reze is a weapon who was a child soldier and never experienced a typical life, and in Denj she saw someone like her.

In fact, unlike Makima, Reze fell in love with him. The final proof of her feelings was her decision to return to the cafe from the train station at the end of the arc, well aware of what she was about to face. She still chose Denji over her mission or even her life, and the tragedy of their lost connection is the most moving situation in the series. She was the one who really fell the hardest, and it proved that it is still possible to find a true heartbeat towards another, even in a place of survival horror.
