How to use Hernand Refinement Token in Crimson Desert
To use Hernand Refinement Token in Crimson Desertplayers will need go to Turnali, the blacksmith in Hernand City. Go into the refinement section and select an armor or weapon you want to upgrade. Once you choose it, you have an option to either upgrade it with a duplicate, spend on crafting materials, or use a Hernand Refinement Token.
Select the Hernand Refinement Token under the Refinement Tokens tab and press the “Refine” button to upgrade your selected equipment by one level. This essentially comes circumvent the cost of manufacturing materials to refine an object.
The Hernand Refinement Token is an additional way to refine your gearand not a replacement for the current upgrade system. You can avoid using tokens and still use materials to refine/upgrade your equipment Crimson Desert.
While it's a useful item, there are a few things about the Hernand Refinement Token that you need to be aware of.

Each use of the Hernand Refinement Token increases your equipment level by one.

Only gear below level 4 can be refined with the Hernand Refinement Token.

If you have an equipment level of 4 or higher, the option to use the Hernand Refinement Token will not appear in the sidebar.
How to get Hernand Refinement Token in Crimson Desert
Hernand Refinement Token can be earned Crimson Desert by completing various main quests and faction quests within Hernand. Players can view the quest's rewards by going to the Journal tab, and the Hernand Refinement Token should appear at the end of the list (if the quest rewards it).
Since the token belongs to Hernand, your best bet is to complete Hernand-based main quests and then focus on side quests to get as many Hernand Refinement Tokens Crimson Desert. There isn't a vendor that sells tokens yet, but that might be a possibility in a future patch.
If you have already completed a quest that offered the Hernand Refinement Token as a reward before Crimson Desert patch 1.01, you will still get tokens in your inventory.
